Membership Declaration & Consent

I hereby declare the above information to be true and correct including that I am 18 years or older and agree to be bound by the Club constitution and by-laws.

I consent to receive marketing materials and other offers from the Club including but not limited to prize draws, promotions, entertainment food, beverage, gaming and the members Rewards program. At any time, I will be able to opt out of marketing communications.

I am aware that:

Proof of age/verification may be called upon at anytime at the discretion of Management,

Replacement membership cards may incur a fee at the discretion of Management.

Player activity statements are available upon request.

My membership remains ‘provisional’ until approval by the Board of Directors.

My membership may automatically renew for 1 year if enough points are available on my membership card.

Membership is not transferrable and the fee, or any part thereof is non-refundable.

I agree to the participation in the loyalty program is subject to the loyalty program Terms and Conditions (as amended from time to

time).

If I currently am or am to become an employee of the Club my membership will change to an Employee Membership and must abide by the Employee Membership policy as listed in the Employee Handbook.
